Application for a Police Criminal Record Certificate (PCRC)
Please read the following instructions regarding the necessary steps to take in applying for a PCRC. Be well aware that the ways for domestic and overseas applications are different.
*Overseas applications can be made BY MAIL
Application requirements for a PCRC are as follows:
(1) A completed application form
(2) Proof of identity: a copy of the passport or an Alien Resident Certificate that is authenticated or notarized by ROC embassies or consulates, Straits Exchange Foundation or other relevant notary public abroad.
(3) A self-addressed envelope
(4) Fee: US$7 is charged for one copy of PCRC(including postage). Each additional certificate applied at the same time is US$1(the contents of each PCRC should be the same)

*Domestic application can be made in person, by mail, via the Internet, app, or by fax.
Application requirements for a PCRC are as follows:
1. To apply in person:
(1) A completed application form.
(2) Proof of identity:
A. R.O.C. citizens: an original R.O.C. ID. An original or copy of the passport photo page is needed for a PCRC with an English name on it.
B. Citizens of China, residents of Hong Kong or Macau: an original Exit & Entry Permit for the Taiwan Area of the Republic of China or a Permanent Resident Certificate.
C. Foreign nationals residing in Taiwan: an original passport or Alien Resident Certificate (ARC)
(3) Fee: NT $100 is charged for one copy of this certificate. Each additional copy applied at the same time is NT$20.
Any minor applicant under age 18, should also present his/her legal guardian’s letter of consent and original ID with his/her application, or have his/her legal representative file the application in person.
2. To apply by mail:
(1) A completed application form
(2) Proof of identity:
A. R.O.C. citizens: an original R.O.C. ID or Household Registration Transcript issued within 1 month
B. Citizens of China, resident of Hong Kong or Macau: an original Exit & Entry Permit for the Taiwan Area of the Republic of China or a Permanent Resident Certificate.
C. Foreign nationals residing in Taiwan: an original passport or Alien Resident Certificate (ARC)
(3) Fee: NT $100 is charged for one certificate of this certificate in cash(Each additional certificate applied at the same time is NT$20.)
(4) A self-addressed registered mail envelope with sufficient postage

3. To apply via the Internet:
(1) Apply on the National Police Agency application page (Application Item: Police Criminal Record Certificate Website Link), and then call 886-2-2381-7494 to confirm.
(2) Please present proof of identity as described and an application fee of NT $100 is charged for one copy of this certificate when picking up a PCRC at TCPD on the assigned day. (Each additional copy applied at the same time is NT$20.)
(3) If applicants DO NOT pick up certificates by the deadline, they will be required to apply again.
4. To apply by fax:
(1) A complete application form
(2) Please fax required documents to 886-2-2381-7487 and then dial 886-2-2381-7494 to confirm application.
(3) Please present proof of identity as described and an application fee of NT $100 is charged for one certificate of this certificate when picking up a PCRC at TCPD. (Each additional certificate applied at the same time is NT$20.)
Notes:
1. If you assign an individual to apply on your behalf, you must submit a valid power of attorney with your signature or stamp included on the application form. The assigned individual must present proof of identity when picking up a certificate.
2. An applicant's English name on the certificate MUST be identical to the one shown in the passport.
